Individualized Services

Training in or engagement and interaction with functional skills and appropriate behavior related to an individual's:
  - Health and safety
  - Personal care
  - Activities of daily living
  - Use of community resources
  - Community Integration

- Medical Assistance with:
  - Medication management
  - Monitoring the individual's health
  - Nutrition
  - Physical condition

- Individual Assistance with:
  - Personal care
  - Activities of daily living
  - Use of community resources

- Life skills training

- Cognitive rehabilitation

- Specialized supervision to ensure the individual's health and safety

- Supported Living Residential Tier 1-4 and/or Customized Supported Living Residential Companion Services:
  - Respite Care services
  - Independent living support services

Specific activities of our Supportive In-Home Services

  • Training in functional skills related to personal care activities (toileting, bathing, and grooming; dressing; eating; mobility; communication; household chores; food preparation; money management; shopping, laundry, etc.).

  •  Training in functional skills related to the use of community resources (transportation, shopping, restaurants, social and recreational activities, etc.).

  •  Monitoring health and physical condition and assistance with medication or other medical needs (or both).

  •  Assistance with personal care and use of community resources, such as (not all inclusive):

    •  Completing personal care tasks when physically unable to learn to do so.

    • · Ensuring hygiene and eating needs are met, such as shaving or brushing teeth; or

    • · Completing daily tasks, such as laundry, meal preparation, using the bank, or other tasks essential to the individual’s health and welfare.

  •  Assistance with arranging transportation to and from training sites and community resources.

  • Specialized supervision to ensure individual’s health and safety:

    • Training in adapting behavior for home and community environments, such as (not all inclusive):

    • Redirecting anger that has been displayed toward others.

    • Handling social encounters with others; and

    • Developing a circle of supports.

COVID 2019 STATEMENT/DISCLOSURE 

 

Due to the COVID-19 pandemic and the nature of the supportive in-home service - where staff will be entering individuals’ homes to work, Person 2 Person, LLC will be continuing to apprise and implement our protocols in response to the COVID-19 pandemic updates.  These protocols are implemented to minimize the risk of exposure to infectious diseases posed by COVID-19 and will remain in effect until further notice.  A discontinuation notice will be forwarded to Department of Behavioral Health and Developmental Services (DBHDS) when these protocols are discontinued.

 

These protocols are implemented in accordance with regulatory guidance received from the Department of Behavioral Health and Developmental Services (DBHDS) and publicized by the Centers for Disease Control and Prevention (CDC).
